Dodgy cam followers/tappets

Good evening I’ve been investigating a worrying engine knock
While preparing to remove the cylinder head on my 1935 14hp (6 cylinder) I removed the tappet cover and luckily (very luckily!) spotted a cam follower socket at a “jaunty” angle. The top was loose. (See photos). Has anyone had a similar experience?
My brief investigations suggest that this is not uncommon and that there are (same part number) improved designs.
